New client Information

"How can my child be seen for Physical or Occupational Therapy services?"

For families seeking Occupational and/or Physical Therapy Services 

Here's what we will need to be added to the waitlist: 

  •  a prescription (or written referral)

A prescription (or written referral) from your child’s physician for the evaluation and/or therapy services. Please contact your medical provider directly to request the prescription to be faxed to our office at 541-770-9212. It will need to include the following:


-The diagnosis and diagnosis code. (ICD-10 code)
-The frequency and duration (For example; 1-2 times weekly for 12 months)


Without the following information above we will be unable to proceed further as insurance will reject it and deny requests for visits.

  • Completed intake form

Submit the online Intake Form. Once both your Intake Form and the referral are received, you will be added to our waitlist.

When you are near the top of our waitlist, our Scheduling team will contact you to follow up with required paperwork.

On the day of your evaluation appointment, check in 15 minutes before your scheduled appointment time. Please bring with you any other relevant reports such as recent therapy evaluations, IEP/IFSP, or mental health evaluations.

To access the required paperwork, please take a moment to complete our online Intake Form by clicking the button below to get started. โ€‹

When you are near the top of the waitlist, our Scheduling team will contact you to follow up with required paperwork. This paperwork packet MUST be completed before an appointment can be scheduled. You are always welcome to call our clinic to check on our waitlist status or notify us if you have found services elsewhere, so we may close your referral.

Insurance and billing

Insurance companies can help cover the cost of our services, and while we are not able to guarantee coverage, we are committed to helping you understand your plan's benefits and assist in utilizing the existing benefits available. We are in-network with many commercial and state insurance plans and do bill services provided at our clinic, even if we are out-of-network.

Financial Assistance is available upon request. Please contact our office for more information.

Referrals

Thank you to the physicians, professionals, teachers, and friends who refer to our clinic! We consider your referral to be the highest compliment and look forward to the opportunity to care for those you are referring!

If you are a Referring Physician who will also be providing the written referral or prescription for occupational or physical therapy, please fax our office at 541-770-9212.

It will need to include the following:โ€‹                                                                       

  • clients name 

  • date of birth

  • parent/guardian's name (if applicable)

  • phone number

  • type of insurance

  • diagnosis and diagnosis code

  • reason for referral  

  • physician's name

  • facility/clinic name

  • contact person

  • phone number

Many referring physicians fax over a "referral form" or chart notes from their EMR program which typically includes the details we need. If it is more helpful to use our Referral Form, we have made one available for you below.

Once we receive your referral, we will contact the client(s) to get the process started.
