New client Information

"How can my child be seen for Physical or Occupational Therapy services?"

For families seeking Occupational and/or Physical Therapy services for their children we need the following:

 1.a prescription (or written referral)

A prescription (or written referral) from your child’s physician for the evaluation and/or therapy services. Please contact your medical provider directly to request the prescription to be faxed to our office at 541-770-9212. It will need to include the following:


-The diagnosis code. (ICD10 code)
-The frequency and duration (For example; 1-2 times weekly for 12 months).


Without the following information above we will be unable to proceed further as insurance will reject it and deny requests for visits.

2. Completed Paperwork

To access the required paperwork, please complete our online Intake Form below. After submitting the Intake Form, you will promptly receive an automatic email with further instructions and the appropriate paperwork for completion.

If you are unable to fill out the paperwork digitally, we are able to mail the packet upon request. They can then be returned by fax (541) 770-9212 or mailed to our office at 400 Crater Lake Avenue, Medford, OR 97504.

Insurance and billing

Insurance companies can help cover the cost of our services, and while we are not able to guarantee coverage, we are committed to helping you understand your plan's benefits and assist in utilizing the existing benefits available. We are in-network with many commercial and state insurance plans and do bill services provided at our clinic, even if we are out-of-network.

Financial Assistance is available upon request. Please contact our office for more information.
